As a partner in the SP-EU project, Wroclaw Medical University contributes its expertise in integrating healthcare and social care services. The project focuses on implementing innovative social prescribing models for people in vulnerable situations, including older adults, migrants, and the LGBTIQ community. Wroclaw Medical University is responsible for research on implementing the Social Prescribing model within the Polish healthcare system, assessing its effectiveness and impact on patients’ quality of life. The university’s participation in the project highlights its leading role in shaping modern, integrated, and accessible healthcare in Europe.
